Use this code when documentation clearly states "post-term pregnancy" or indicates a gestational age of 42 weeks or more. This diagnosis is often made when labor has not spontaneously begun by this point, or when induction is being considered or performed due to the prolonged gestation.

• Do not confuse with "prolonged pregnancy" (O48.1), which is 40-41 completed weeks.
• Always confirm the gestational age documented in the medical record.
• Sequence as a principal diagnosis when the reason for admission or encounter is the post-term status.
