Threatened abortion

This code signifies a pregnancy at risk of spontaneous termination before 20 weeks of gestation, characterized by vaginal bleeding, with or without abdominal pain, but with a closed cervix and no expulsion of products of conception. The fetus is still viable, and the pregnancy may continue.

Apply this code when a pregnant patient presents with symptoms such as vaginal bleeding or cramping, and ultrasound confirms a live intrauterine pregnancy with a closed cervical os. Documentation should clearly state "threatened abortion" or "threatened miscarriage" and rule out inevitable or incomplete abortion.

• Differentiate from inevitable abortion (O02.1) where the cervix is dilated.
• Ensure documentation specifies a live fetus; if fetal demise has occurred, a different code for missed abortion (O02.1) or blighted ovum (O02.0) may be more appropriate.
• Sequence as the primary diagnosis when the threatened abortion is the reason for the encounter.
