Presbyopia

Use this code when documentation indicates a patient is experiencing age-related farsightedness, typically presenting in middle age, and requiring corrective lenses for near vision tasks. This diagnosis is supported by an ophthalmological or optometric examination confirming impaired accommodation.

• Do not confuse with hyperopia (farsightedness due to refractive error), which is coded separately.
• Often co-occurs with other refractive errors; code all documented conditions.
• Ensure the documentation clearly states "presbyopia" or describes the characteristic age-related loss of near vision.
