Delusional disorders

This code represents a mental health condition characterized by the presence of one or more non-bizarre delusions that persist for at least one month. These delusions are typically well-systematized and do not significantly impair psychosocial functioning, nor are they due to the physiological effects of a substance or another medical condition.

Use this code when documentation clearly indicates a primary diagnosis of a delusional disorder, such as erotomanic, grandiose, jealous, persecutory, or somatic type, without other prominent psychotic symptoms or mood disturbances. Supporting documentation should detail the specific nature and duration of the delusions and rule out other psychiatric conditions.

• Carefully review documentation to differentiate from mood disorders with psychotic features; these are Excludes1 conditions.
• Ensure the patient's primary symptom is persistent, non-bizarre delusions, not disorganized speech, hallucinations, or negative symptoms typical of schizophrenia.
• Do not confuse with paranoid personality disorder (an Excludes2 condition), which involves pervasive distrust and suspiciousness but not fixed delusions.
