Assign this code when documentation clearly states the patient is experiencing suicidal thoughts, regardless of whether a specific plan has been formulated or an attempt made. This is often identified during psychiatric evaluations, mental health screenings, or crisis interventions.

• Always sequence this code as a primary diagnosis when it is the reason for the encounter.
• Do not confuse with actual suicide attempts (T14.91XA) or self-harm behaviors without suicidal intent.
• Ensure documentation specifies "ideation" and not just general depression or anxiety, unless suicidal ideation is explicitly stated as a symptom.
