Paralytic ileus, also known as adynamic ileus, is a temporary cessation of intestinal peristalsis without mechanical obstruction. This condition results in the accumulation of gas and fluids within the bowel, leading to abdominal distension, nausea, vomiting, and absent bowel sounds. It is a functional disorder of the intestines, not a physical blockage.

Use this code when documentation clearly indicates a diagnosis of paralytic ileus, adynamic ileus, or non-mechanical ileus. This often occurs post-operatively, after trauma, or in the context of electrolyte imbalances, medications (e.g., opioids), or severe illness. The physician's diagnosis of "paralytic ileus" or "postoperative ileus" directly supports the use of this code.

• Differentiate carefully between paralytic ileus (functional) and mechanical obstruction (physical blockage), as they are coded differently.
• Review documentation for the underlying cause of the paralytic ileus, as this may require additional codes.
• Do not code this with gallstone ileus or any other type of obstructive ileus due to the Excludes1 note.
