Sexual sadism

This code represents a paraphilic disorder characterized by recurrent, intense sexually arousing fantasies, urges, or behaviors involving inflicting physical or psychological suffering on another person. The individual experiences significant distress or impairment in social, occupational, or other important areas of functioning, or has acted on these urges with a nonconsenting person.

This code is appropriate when a patient meets the diagnostic criteria for sexual sadism, typically established through a psychiatric evaluation. Documentation should clearly indicate the presence of recurrent sexual arousal from inflicting suffering, and either personal distress/impairment or nonconsensual acts.

• Ensure documentation specifies distress, impairment, or nonconsensual acts, as paraphilias are only coded when these criteria are met.
• Do not confuse with masochism (F65.51), where arousal is derived from being humiliated, beaten, bound, or otherwise made to suffer.
• Always sequence this code as a primary diagnosis when it is the focus of treatment.
