Disseminated zoster

This code represents a severe form of herpes zoster (shingles) where the varicella-zoster virus spreads beyond the primary dermatome, affecting multiple non-contiguous dermatomes or involving visceral organs. It indicates a systemic infection rather than a localized rash.

Use this code when documentation explicitly states "disseminated zoster" or describes widespread vesicular lesions occurring in three or more dermatomes, or when there is evidence of visceral involvement (e.g., zoster pneumonia, encephalitis, hepatitis). This condition is often seen in immunocompromised patients.

• Differentiate from localized zoster (B02.9) or zoster with specific complications (e.g., B02.21 for zoster meningitis) when the dissemination is not the primary focus or is not explicitly documented.
• Always review the medical record for any associated complications (e.g., ophthalmic, neurological, visceral) and code them additionally.
• Ensure documentation supports the "disseminated" nature; merely having multiple lesions in a single dermatome does not qualify.
